Cases if chronic ankle sprain should be considered seriously as recurrence of such cases may end in ankle instability . also for cases of ankle fractures, ligamentous integrity around ankle joint should be assessed during open reduction and internal fixation of the fracture and if it is cut it should be repaired.
Cases of ankle instability should be managed conservatively for at least 6 months as this therapy gives success rate about 80%.
Those who didn’t found improvement with conservative therapy , direct anatomic repair should be considered as it is less invasive and preserves the normal anatomy .
If the end of the ligaments are torn enough that couldn`t under go repair or there is deformity of the bone ,e.g, cavus deformity or tarsal coalition or there is generalized ligament laxity the second choice will be minimal invasive technique using polyester tape as it is short operation done under small incision and avoid sacrifice other structures and needs short post operative immobilization.
Summary and conclusion Finally if there’s contraindication for direct repair or the polyester tape is not available non anatomical repair which depends on harvesting other tendons for stabilitization of the ankle is considered , taking in mind possible complication that liable to occur eg , sural nerve injury , graft side morbidity that may affect of ankle biomechanics , furthermore the incisioin required is also substantially longer than other techniques .
