Abstract

Obesity is a metabolic disease in which adipose tissue comprises a greater proportion of body tissue than normal. It affects up to 33% of the population in North America Almost 5% of obese patients weight more than double their, ideal body weight as predicted by age, sex , height. The precursors to obesity are multifactorial and include a genetic tendency, environmental effects, education, sex, race and socio-economical status . Most of the major obesity-related health risks increase with increasing weight. The rate of premature death is increased in obese patients and doubles in populations weighing 40%-60% more than ideal body weight/4’5)Obesity results in multiple pathophysiological disturbance including cardiovascular, respiratory, gastrointestinal, metabolic, pharmacological changes, and hormonal changes. These changes complicate perioperative anaesthetic and surgical management of obese patients. Consequently. postoperative morbidity and mortality are higher in obese than in non obese patients.Morbid obesity has a profound effect on ’respiratory function, lung volumes, compliance, functional residual capacity, and arterial oxygenation.Also the site of surgery affects respiratory function (impairment of respiratory function is greater after abdominal surgery than after non-abdominal surgery).
