Incisional hernia is the second common type of hernia after inguinal hernia. An incisional hernia is one which develops in the scar of surgical incision. Irrespective of contributing causes to the development of incisional hernia, it is fundamentally a failure of the approximated musculoaponeurotic and fascial layers to remain in apposition. Repair of large abdominal wall incisional hernia is a difficult surgical problem with recurrence being a common complication, in addition to other complications such as hematoma, seroma and sinus formation. Primary repair of incisional hernia with or without lateral relaxing incisions is associated with recurrence rate approaching 30%. This recurrence rate increases to 44% for similar repair of recurrent incisional hernias. It is well known that one of the principal causes of recurrence is tissue weakness which is a fact supported by surgical observations and clinical experience. Principally, hernioplasty is essential in repair of large incisional hernia otherwise recurrence will develop. The mesh used adds strength to the already weak abdominal musculature which makes a support against the increased intra-abdominal pressure that develops after sudden reduction of the hernial contents into the abdominal cavity.
