Abstract

Background: In view of the high mortality associated with ischemic heart disease (IHD), the estimation of individual cardiovascular risk over and above the assessment of classic risk factors, such as age, hypercholesterolemia and hypertension, is an important prerequisite for focusing preventive measures and therapeutic measures. Microalbuminuria (MA) is not only a predictor of diabetic complications but also a powerful independent risk factor of cardiovascular disease (CVD).
Aim: The aim of this study was to investigate the association between Microalbuminuria and cardiovascular disease in diabetic patients referred to SPECT unit in Ain Shams University Hospital.
Patients and methods: Fifty patients with type 2 diabetes (27 men and 23 women, aged 42-75 years), with a negative history for infarction and negative clinical or ECG findings of CAD, were included in the study. Coronary artery disease was assessed by positive myocardial TC99 Sestamibi Scintiscan using the SPECT method (TC-scan) .Urinary albumin in first morning sample was estimated by immunoturbidimetry method. Albumin to creatinine ratio (ACR) was calculated as mg/g. In addition, blood pressure, total cholesterol level, body mass index (BMI) and smoking habits were recorded. Results: Among the 50 studied patients ,we found that microalbuminuria was present in 19 patients (38 %). Among the group with positive SPECT (30 patients) , we found MA in (16) patients ( 53.3% ) .Among the group with negative SPECT ( 20 patients ) ,we found MA in ( 3 ) patients (15%). Among the 19 patients with MA , we found that 16 patients had positive SPECT (84%).Among the 31 patients without MA ,we found that 14 patients had positive SPECT (45.2%).
Conclusion: Microalbuminuria is common among diabetic patients with ischemic heart disease.
