Abstract

Diabetes Mellitus (DM) is a syndrome of metabolic disease characterized by hyperglycemia. In Egypt, the incidence of IDDM is 8.3/100000 in urban areas and 7.6/100000 in rural areas.
Over the past two decades, quality of life has emerged as an important factor in the investigation of patient responses to illness experiences and therapeutic interventions. Diabetes in children and adolescents poses serious physical, mental and emotional challenges. This is clearly due to fundamental conflicts existing between the tasks of managing diabetes and the normative development facing the young adolescent.
One critical component of initial diabetic care is patient education. Diabetes education has an overall beneficial impact on health and psychosocial outcomes. Also, it helps adolescents to avoid complications.
Aim of the study:
The study was conducted aiming to assess quality of life, its determinants and its relation to glycemic control among diabetic adolescents in Alexandria and to design, implement and evaluate an educational program for these adolescents.
Target population and study sampling:
To fulfill this aim, the study was conducted on 503 diabetic adolescents chosen randomly from diabetes out patient clinics in El-Shatby hospital and Sporting Student’s hospital. A cross-sectional design was used to study QOL, its determinants and its relation to glycemic control among those adolescents. An intervention education program was implemented on 102 adolescents having poor adherence to diabetes management.
Technical design:
For the execution of the study:
A- A structured interview questionnaire was used to collect data from adolescents about:
1- Sociodemographic characteristics and social family environment of the patients.
2- Medical history of diabetes mellitus.
3- Knowledge of the adolescents about diabetes and data about adherence to self-care.
4- Self-efficacy of diabetics about aspects of self-management. Self-esteem of adolescents, depression and family contribution in management were also asked for.
5- The diabetic QOL scale for adolescents developed by Ingersol and Marrero was used after being translated to Arabic and tested by a pilot study.
B- Review of medical records to obtain: glycosylated hemoglobin level in the last six months, type and doses of insulin used, complications of the disease.
C- A quasi-experimental design was used to construct and implement an educational intervention program for diabetic adolescents having poor adherence to diet, glucose monitoring and exercise. The diabetics were randomly allocated into two groups
