Abstract

Nowadays, increasing attention of most of administrators has been focused on improving work environment and staff nurses’ job satisfaction. Structure that promotes decisional involvement as participative management and shared governance increases job satisfaction and performance, and decreases turnover. Nurses’ involvement in decision-making supports personal and professional integrity while optimizing patient care, and may also contribute to feeling of job satisfaction. Lack of decision-making involvement for nurses has been related to nursing burnout.This study was conducted in order to determine to what extent staff nurses are involved in decision-making, and its effect on their job satisfaction in different units at Menoufyia University Hospital. This was achieved through measuring the involvement of staff nurses in decision-making, identifying to what extent staff nurses are satisfied with their job, and then identifying the relationship between nurses’ decisional involvement and their job satisfaction. The units and departments included in the present study from Menoufyia University Hospital were emergency, medical,surgical, pediatric, obstetrics and gynecology, orthopedics,ENT, urology, ophthalmology, and neurology departments, in additional to oncology, pediatric intensive care unit, kidney dialysis unit, premature unit, burn unit, and intensive care units.The subjects were 241 staff nurses with nursing school diploma or bachelor degree in nursing, in addition to all 19 head nurses of the units in which the study was conducted.Two kinds of tools were used in this study in order to answer the research questions. These were namely decisional involvement scale and the work quality index Both DIS and WQI were translated into Arabic. A pilot study was conducted on twenty nurses, and necessary modifications were done.The main findings of the study revealed the following: The highest percentage of staff nurses were aged less than25 years were nursing school diploma and having less than 5 years of experience in nursing About one third have expressed their willingness to quit current hospital during the next 5 years In general, of the study staff nurses reported a shared decision making between nursing management and staff nurses. About two thirds of them reported reported a low staff nurses’ decisional involvement concerning professional recruitment, whereas the highest percentage of staff nurses’ decisional involvement was related to collaboration liaison activities The highest percentage of staff nurses who reported shared decision-making between nursing management and staff nurses were at surgical departments. A low percentage of staff nurses were satisfied with their job. The highest satisfaction was with professional relationships and lowest concerning professional role enactment A significant positive correlation was revealed between staff nurses’ decisional involvement and their job satisfaction scores at different units in MenoufyiaUniversity Hospital No statistically significant correlations could be revealed between staff nurses’ decisional involvement and their age or years of experience A significant positive correlation was revealed between age and job satisfaction, with no correlation with years of experience. Half of the study nurse managers reported ashared decision making between nursing management and staff nurses. There were no differences between staff nurses and nurse managers’ perception concerning staff nurses’ decisional involvement. Job satisfaction scores were positively predicted by nurse’s age and decisional involvement score, and negatively predicted by experience years.In conclusion, the involvement of staff nurses in decisionmaking concerning administrative issues has a positive effect on their job satisfaction. It is recommended that in-service education programs be conducted for head nurses, with emphasis on how to utilize and use skills and experiences of professional nurses to be involved in decision-making. Lastly,decision-making as a theoretical body of knowledge must be included in undergraduate and postgraduate studies to enhance nurses’ knowledge regarding decision and how it could be made.
