Abstract

High risk clients are those who have known conditions that will, or can, have a strong potential to compromise the health of the pregnant woman, her fetus, or both. Home visits by nurses enable the client to remain safely in her own environment during the experience of a high risk pregnancy care for this population includes the provision of physical care, home fetal monitoring, education, and support through out the pregnancy.Therefore, this study was conducted for identification, early detection of high–risk pregnancy and prevention of complications.The study aimed to explore the incidence of pregnancy-related comlications; and the effect of home visits on early detection and identification of high risk pregnancy and prevention of complications.The study was conducted at four maternal health centers (MCHCs) in Shebeen-Elkom. The selected MCH centers are considered to cover different places in Menoufiya Governorate: Meleeg, Ganzour, El Erakia And Shobra Bloula. A convient sample of pregnant women who were attending at four villages: Meleeg,Ganzour, Shobra Bloula and EL Erakia MCH centers. Total number of study sample comrised 300 pregnant women who did not identify medical disorders. The sample was assigned into four groups each group consisted of 75 pregnant women from each village Data collection tools included•
Interviewing questionnaire,which contained question about socio-demographic data,such as age level of education,inaddition to questions related to personal history, as well as past and present obstetric historyAssessment of Antenatal Sheet, which contained assessment of the mother from the head to teos,vital signs,and obstetric examination Follow up sheet •Assessment of postpartum sheet Assessment of neonatal sheet.The field work lasted for about eight months for data collection,from September 2001 to May 2002. The time consumed with each woman 20-30 minutes at initial visits then 15-20 minutesduring follow up visits.According to sociodemographic characteristics of the sample, age ranged from 18-40 years,with the majority of the women’ age ranged 20-25 years. Regarding to level of education of the studied sample had high school education, 13.7% had highly educated, were be able to read and write, illerate. As regard to parity of the studied sample were multipara,while were grandmultipara, and were primipara The most important finding of this study, as regards to number of visits of the studied sample were visited by researcher 7 timesduring pregancy.However, of the studied group had no both major and minor complaints during pregnancy. But of the studied group had both major and minor complaints during pregnancy. Furthermore, as regards to causes of referal 15.3% of studied group were referred due to hypotension,were referred due to hypertension and vaginal bleeding, were referred due to leaking of fluid, were referred due to PIH. Almost half of deliveries of the studied subjects were conducted at home,while delivered at a hospital, while delivered at a private clinic.In relation to the problem of breast problem of studied groups had cracked nipple, of studied groups has, had breast engorgement, has mastitis in Ganzour village. There is no statistically significant difference.
In relation to birth attendant the study also shown that 58.3% of the delivery attend by a physician, attend by nurse, attend by daya, attend by relative.As regards to postpartum complications88%of the sample had no complications, 0.3%had pph and postpartum infection, had PIH, had obstructed labor, had prolonged labor.As regards to neonatal complications of the sample had no complications,had hyperbilirubinamia, had congenital anomaliesand died.As regards to birth weight,of the neonatal weight were within normal range 3-3.5 kg; their neonatal weight was 2.5-3 kg their neonatal weight was 3.5 kg, their neonatal weight was 2.5kgThe study recommened that Governments and health providers need to recognize that every pregnancy is special and should ensure that all pregnant women have access to high-quality maternal health services by:Educating women and their families about the risk of complications faced by all women, and about actions they should take if and when a problem arises. Providing adequate care as close as possible to where women live.
